Branch office address
Self-attested copy of residence or identity proof
If a major mismatch exists, you may also need an affidavit from the nearest court, along with valid identity or residence proof.

Download and print the Demat modification form from your bank or DP’s online portal and enter signature modification details.
Sign using both your old and new signatures to notify the bank. Complete your KYC form and sign across your photograph.
Obtain an acknowledgment letter from your bank with the banker’s signature and seal.
Send these documents to your DP along with valid identity and address proof. Make sure all documents are self-attested with your new signature.
Remember, your shares will no longer be issued in physical form. Once your documents are accepted, the shares will be transferred directly to your Demat account. This makes having a Demat account a necessity.
Your bank or DP will charge a minimal processing fee along with applicable GST for signature modification.
